A change with a purpose

Each action describes what should change, why it matters and which dependencies must be considered. Separate immediate safeguards from structural work so the team can discuss a realistic sequence.

Responsibility and a completion criterion

Suggested owners are agreed with your team. State what completion means before the work starts: a behaviour that must change, a control that must operate or evidence that must be produced.

Verification after implementation

Define a check that addresses the original issue and confirms the expected behaviour still works. Record the result and any remaining limitation. Implementation and follow-up verification can be scoped separately from the review.
